Hockey coaches at Arizona State University (ASU) didn't submit a plan for review by trainers before an outdoor workout on a hot August day that saw Hamilton-born NHL prospect Matthew Mayich collapse, leaving him in hospital in critical condition, according to a review by the Tempe-based school.
